I acknowledge that I have enrolled in a Temporary Traffic Management (TTM) training program delivered by Carlisle Learning & Development.

By signing this form, I authorise Carlisle Learning & Development to collect, use, and disclose my personal information, training records, assessment outcomes, and qualification details to relevant authorities and industry bodies for the purposes of:

• Recording and verifying the successful completion of Temporary Traffic Management (TTM) Skill Sets and associated training programs.

• Reporting training and assessment outcomes to Austroads and the South Australian Department for Infrastructure and Transport (DIT), where required.

• Facilitating the issue, renewal, replacement, or management of my Traffic Management Card.

• Maintaining and updating records of my qualifications, competencies, certification status, and expiry dates within applicable national, state, or industry-based databases and registers.

• Meeting regulatory, contractual, compliance, auditing, and reporting obligations associated with TTM training and accreditation.

I understand that the information disclosed will be limited to that required for the administration, verification, and management of my TTM qualifications and accreditation and will be handled in accordance with applicable privacy legislation and information management requirements.

I confirm that the information I have provided is true and correct and that I voluntarily provide my consent for Carlisle Learning & Development to release this information for the purposes outlined above.
